Physical and Dental Exams
Every child of school age attending or who should be attending a public or non-public school is entitled to the following services provided by the local public school district.  Exams will be done during the school year.  You may choose to have your child seen by your own physician.  Exams may be completed during the summer prior to the start of school.
§        Physical Exam             Grades K, 6, 11
§        Dental Exam                Grades K, 3, 7

The following forms are provided for you: 
§         Private physician exam form (2 pages) and
§         Private dental exam form
