Dental exams are required by the Pennsylvania Department of Health to be completed in 7th grade. It is recommended that this examination be completed by your family dentist as he/she can best evaluate your child's dental needs and assist you in obtaining necessary treatment.
Forms are available to be printed below or may be picked up in the nurse's office.
Dental exams that are completed with in 1 year prior to the first day of the school year will be accepted.
