Physical exams are required by the Pennsylvania Department of Health to be completed in 11th grade. It is recommended that this examination be completed by your family physician as he/she can best evaluate your child's health and assist you in obtaining necessary treatment.
Physical exams completed with in 1 year prior to the first day of the school year will be accepted.
Forms are available to be printed below or may be picked up in the nurse's office.
