Immunizations

"What vaccinations are required before my child starts Kindergarten?"

Tetanus, diptheria, acellular pertussis
(DTP, DtaP, DT or Td)
4 doses
** 1 dose must be given on or after 4th birthday

Polio
(OPV, IPV)
4 doses
**4th dose must be given on or after 4th birthday and at least 6 months after previous administered dose

Measles, Mumps, Rubella
(MMR)
2 doses

Hepatitis B
3 doses

Varicella
(Chicken Pox)
2 doses
**If child had the Chicken Pox Disease, evidence of immunity required

IMPORTANT: If a child does not have all the doses listed above, needs additional doses, and the next dose is medically appropriate, the child must receive that dose with in the first 5 days of school or your child has a risk of exclusion from school.

"What if my child has a medical or religious exemption for vaccinations?"

If your child has a medical exemption for vaccinations, a signed form from the physician must be obtained and turned into the nurse's office.
Every school year, your child's immunization card will be sent home for signature by physician for medical exemption or parent/guardian stating religious exemption.
